The healthcare system is in crisis, and thousands of Kaiser Permanente workers are taking a stand. Over 31,000 employees have been on strike for four weeks now, demanding better wages and staffing conditions, and their actions are sending shockwaves through California and Hawaii. But here's where it gets controversial: while patients face disrupted appointments and delayed treatments, Kaiser argues it’s already offering competitive pay. So, who’s really to blame for this standoff?
This isn’t the first time Kaiser has faced major labor disputes. In 2022, mental health workers walked out for 10 weeks, and in 2023, a strike required federal mediation. This time, the United Nurses Associations of California/Union of Health Care Professionals—representing nurses, physical therapists, midwives, and more—are at the forefront. It’s their first-ever strike, and they’re demanding a 25% raise over four years to combat inflation and staffing shortages. Kaiser counters with a 21.5% offer, claiming its employees are already among the highest-paid in the industry. And this is the part most people miss: Kaiser sits on $66 billion in reserves, yet it calls using those funds for wages ‘financially irresponsible.’
Workers accuse Kaiser of violating staffing agreements and compromising patient care—claims the healthcare giant denies. Union leaders argue that Kaiser can afford the wage increase, especially after rebounding from a $4.5 billion loss in 2022 to a $12.9 billion net income in 2024. But Kaiser insists its reserves are for long-term commitments and emergencies, not payroll. The union’s proposal would cost an additional $1 billion, which Kaiser says could jeopardize member premiums.
Joe Guzynski, the union’s executive director, points out that other Kaiser unions received inflation-adjusted raises after 2022. ‘We’re asking for the same deal,’ he says. ‘It’s about restoring fairness.’ The union is also advocating for three newly formed groups in Northern California—certified nurse midwives, anesthetists, and physician assistants—who are fighting for their first contracts. Kaiser has proposed cutting retirement benefits, freezing wages, and slashing pay for new hires, sparking outrage among these essential workers.
Nurse midwives, who deliver 80% of vaginal births in Kaiser’s Northern California hospitals, say their work reduces cesarean sections, maternal complications, and costs. Yet, they feel undervalued. ‘It’s like being $10 apart for the common person,’ says Brian Mason, lead negotiator for the nurse midwives. ‘But they’re acting like we’re asking for billions.’
For patients, the strike has been devastating. Social media and local news reports are flooded with stories of cancelled chemotherapy, surgeries, and long pharmacy lines. Cecilia Ochoa, a lifelong Kaiser member, shared her ordeal of waiting hours for antibiotics during a urinary tract infection. ‘It was so bad, they were handing out snacks and water,’ she said. While she supports the strikers, she can’t shake the feeling that Kaiser has lost sight of its mission: ‘It’s not about the patient, it’s about the money.’
